ARRANGEMENT OF DOCUMENTS FOR SCREENING

1. Application Form (original)
2. 2018 JAMB Registration Slip – Two copies
3. Testimonial (Primary or Secondary) (original and photocopy)
4. Student Information Form (original)
5. O’Level Certificate(s) (original and photocopy)
6. Birth Certificate/Sworn Affidavit that predates the admissions advertisement.
7. All higher certificates for Direct Entry Applicants
8. NYSC Certificate/NYSC Exclusion Certificate for Direct Entry Applicants
9. Marriage Certificate, Affidavit for Change of Name, and Newspaper Classification for Married female only.
10. Health Service Questionnaire (Coloured)
